Player Registration Process

For new or transferring Players:

Clearance letter from previous club.
2024 MySafa card.
GDLFA registration form in duplicate, signed & stamped by school across photo.
Certified copy of Birth certificate with photo attached and stamped by school.
Juventus Internal Form with signed Indemnity Form. This must be completed and handed in on Sunday or at training.

For renewing players:

Each team must complete a composite form. Players details completed and parents to sign.
2024 MySafa card must be attached.
Each player must complete a single copy of GDLFA registration form which must be stamped and signed by player school and parent must complete consent section and sign.
